Requirements for Passport Renewal<\/p>\n\n\n\n

Following are the Requirements for Passport Renewal in the Philippines:<\/strong><\/p>\n\n\n\n

Brown Passport or issued prior to 01 May 1995<\/p>\n\n\n\n

\n
• Old passport and photocopy of passport pages 1,2,3 (amendment). The pages showing latest Bureau of Immigration departure and arrival stamps.<\/li>\n\n\n\n
• Supporting document with complete middle name.<\/li>\n<\/ul>\n\n\n\n

  MRP & Green Passport or issued after 01 May 1995<\/p>\n\n\n\n

  \n
  • Present passport and photocopy of inside and back cover.<\/li>\n\n\n\n
  • The pages showing latest Bureau of Immigration departure and arrival stamps.<\/li>\n\n\n\n
  • For illegitimate minor, personal appearance of mother is required.<\/li>\n<\/ul>\n\n\n\n

    For married women, who want to use surname of their spouse:<\/p>\n\n\n\n

    • Personal Appearance is required<\/li>\n\n\n\n
    • Marriage Contract (MC) in Security Paper issued by NSO or CTC issued by the Local Civil Registrar duly authenticated by NSO.<\/li>\n<\/ul>\n\n\n\n

      For Minors (below 18 years old): Personal appearance of either parent is required.<\/p>\n\n\n\n

      • If minor is NOT traveling with either parent:<\/li>\n\n\n\n
      • Original and photocopy of DSWD Clearance<\/li>\n\n\n\n
      • Affidavit of Support and Consent<\/li>\n\n\n\n
      • If both parents are abroad:<\/li>\n\n\n\n
      • Affidavit of support and consent (must be authenticated by the nearest Philippine Embassy or Consulate General if not executed before a Philippine Consul).<\/li>\n\n\n\n
      • Special Power of Attorney (must be authenticated by the nearest Philippine Embassy or Consulate if not executed before a Philippine Consul designating the representative by name and authorizing him to apply for a passport on behalf of the minor).<\/li>\n\n\n\n
      • Passport and photocopy of the passport of person travelling with the minor<\/li>\n<\/ul>\n\n\n\n
